Before you complete this form, you will need to open up the FMC directory to see
your current information. Then, fill out
the following form to update your info or
to add a completely new person to the directory. Fields in RED are
required. You must provide your name and a valid email address to submit
this form. Also note, that sending
this information will not automatically update the directory. The church's
administrative assistant will review the information before updating the directory.
You don't have to fill out each field in this form if you are only
updating info. For example, if you are only updating your phone
number, then just fill out the required fields, your household name, and your new phone
number in the top section.
You can add an email address for more than 1 person in your family...if you choose to do
this, use the additional person sections at the bottom part of the form...be
sure to include the name and their email address.
If entering a new family, complete ALL fields in the top of the
form and then fill out the fields for each family member below.
